Our transition support workers provide hands-on guidance and emotional support to help you navigate these changes with confidence. We work with participants, families and other service providers to ensure transitions are smooth, planned and positive.
Our personal care support assists participants with the essential tasks of daily living - including grooming, bathing, dressing, personal hygiene and continence management. We approach every interaction with respect, sensitivity and professionalism, preserving your dignity at all times. Care plans are personalised to your preferences, routines and level of independence, ensuring you feel comfortable and in control.
Reliable transport is essential for independence. We support participants with safe, flexible travel arrangements to reach medical appointments, workplaces, education facilities, social activities and community services. We can also assist participants in building their own travel skills and confidence using public transport, working towards greater independence in getting around their community.
We support participants and families with home modification needs that improve safety, access and independence at home. This may include helping participants understand recommendations, coordinate with builders or allied health professionals, and keep the process aligned with their NDIS goals and funding.
We support participants to understand, access and use assistive products that can improve independence, safety and comfort in daily routines. This may include helping participants follow recommendations, use equipment confidently and communicate product-related needs with their wider support network.
We provide practical daily support for participants in shared living or supported living arrangements. Support may include routines, household responsibilities, personal organisation, communication and day-to-day skill building, depending on the participant's goals and living environment.

Our household support workers assist with practical tasks required to maintain a clean, safe and comfortable home. This may include cleaning, laundry, grocery shopping, meal planning and preparation, depending on each participant's plan, routines and preferences.

We can assist participants with SDA-related enquiries, coordination and day-to-day support needs where Specialist Disability Accommodation is part of their NDIS plan. Our focus is on helping participants understand their options and maintain safe, stable and suitable living arrangements.
